Utilizing the person-centered planning process can help us individualize the transition plan for a student with Complex Support Needs through identifying critical factors that lead to appropriate gainful employment. The challenges we often face are to find, select, and conduct the appropriate assessments with the student with Complex Support Needs as well as select what information from related service providers, parents, and other stakeholders to include in the transition plan. Join us as we discuss strategies to increase your ability to have students with Complex Support Needs actively participate in the assessment process towards the development of the transition plan.
